You need file I-539,download the form from uscis,the instruction will tell you what kind of supporting document you need to apply H4.excepte these documents,your wife also need send following documents:

1.current I-20 indicate your wife applied the OPT.
2.copy of your wife's EAD card.

Do not need Lawyer,it is not hard.
