J-1 waiver?

I came to USA on a J-1 visa which was for two years as shwon in my IAP-66. Toward the end of my first J-1 period, I extended my J-1 for one more year under the same program. Now I am on the extension. I got my J-1 waiver approved during my first J-1 period. I am wondering if I can use this waiver document to apply for a H-1. My concern is that since I am now on the extension, my waiver may not be useful although I am still under the same program.
