我表妹申请婚姻绿卡同时申请的EAD邮寄丢失，移民局说需要从新申请，是同之前一样的I765表格吗？

-Yes. Provide the copy of the I-485 Receipt Notice with I-765 filing

还是有其它替代表格？另外单独重新申请I765需要支付多少费用？

-Check the I-485 Receipt Notice to see when USCIS received your I-485. If she filed I-485 after April 1, 2024, then she needs to pay I-765 filing fee.
