I-130填表问题兄弟姐妹绿卡

请教8老师，给弟弟及他家人申请绿卡，如果他的公司已经给他提交了PERM, 待批中，填i130时，有个问题是，has anyone else ever filed a petition for the beneficiary? 是填yes吗？

Was the beneficiary ever in immigration proceedings? 填yes 还是no？

填I-94 info 时有个问题是 when does the beneficiary's authorized stay expire? I-94 上 Admit until date: D/S 不知道D/S 是什么意思，应该怎么回答？

最后要交official statement, 我已经交了我和我弟弟的birth certificate. 不知道还能交什么？问题比较多，非常感谢老师您的耐心回答！
