-There is no such question on the paper based CBP form. Because this kind of medicine is very common for the common desease, your father may not need to declare it. But he had better prepare the documents for bring the medicine (e.g. the doctor's dianosis for your afther's desease and prescription for the medicne-translate this document to English) in case the CBP officer may ask the questions about the medicine if the officer inspect to find it. He had better keep the medicine in the original container (not open the container). You may write a letter in English for your father to the CBP officer to explain it and your father can present the letter with the medicine to the officer if the officer asks questions.
