1）. Part2: Information about your parents：如果petitioner的父母早已过世，那还用填他们的 City/Town/Village of residency 和 Country of residency 吗?

-No. Just put "Deceased" in the Address part.

2）. 40a~d，是否填 latest entry to the US ?  当时还用的B2入境，是否填当时入境的信息。

-Yes. It is very important to put the lastest entry information there.

3）Part4: Beneficiary's Employment Information, 如果是自雇，Name of current employer 是写自己的bussiness name 还是写 self employed.

-Put the Bussiness name there and put "(self employed)" after the bussiness name, e.g., XXX Comonay (self employed).
