-Probably your birth certificate 公证 is too old, not meet the requirements. You may ask NVC to see what the problem is, and you may need to do a new 出生证明公证 which includes the translation version.
再就是我母亲5年前去世了，需要提供我母亲的死亡证明。
-This document should not be required. Did NVC ask for this document?
我用的医院给我父亲出局的死亡证明，我在美国找了个翻译以后certified translate了一下。
-Probably the translation does not meet the requirement. See this for the USCIS requirement for translation:
