请问老师

为父母申请绿卡， 今天收到NVC的电邮说我（本人是SPONSOR， 老公是HOUSEHOLD MEMBER）需要在864A表的PART5输入我的名字，我不明白，那一页只要求我的签名（31a,  31b）啊。问了家律师事务所，说我这种情况是被CHECK了要重新做。有这么严重吗

请老师们帮我看看864A PART5哪里错了？
