  1. $1,760 personal check ($535 I-130 filing fee + $1140 I-485 filing fee + $85 biometric fee)
  2. Four (2x2) passport-styled photos
  3. Copy of Beneficiary's Passport (Biographical page and visa page)
  4. I-94
  5. Form G-1145
  6. Form I-130
  7. Form I-485
  8. Form I-864
  9. Form I-131
  10. Copy of Notarized Birth Certificate of the Beneficiary
  11. Copy of Notarized Marriage Certificate of Beneficiary
  12. Copy of Petitioner's Naturalization Certificate
  13. Copy of Notarized Birth Certificate for the Petitioner
  14. Copy of Petitioner's Certificate of employment and income
  15. Copy of Petitioner's Recent six months' pay statements, and six months bank statements
  16. Petitioner's Federal tax return transcript for year 2017
  17. Copy of Petitioner's W-2 forms for the last 3 years (Tax years 2015-2017)
  18. Form I-693 from USCIS designated civil surgeon
