1. 要求past five year, 那么最后一个address from 是写5年前的日子，还是写实际的from date

比如，实际居住时间是2000/1/1 - 2016/1/1，可以写2013/7/12 - 2016/1/1吗？因为实际的from date可能记得不是很清楚了-Just estimate the date in 2000. Check your tax returns if possible.

2. 中国的地址好罗嗦，大家都是怎么填的啊，比如3栋2单元13层1号
