朋友的孩子和女儿是从小一起长大的好朋友， 已经在中学修了4-5 年中文，暑假想跟我们回中国。  朋友一家是美国人。  父母不去中国，想让孩子跟我们走。  我记的除了minor traveling consent form 外， 还有一张移民表格要填， 各位大侠知道是什么表格？
- Need to get the  medical consent from the kid parents to give you the right to authorize medical treatment for the child during the travel.
