高龄父母持绿卡六年左右，最近想申请入籍。父亲前几年小中风，语言和记忆都不大行了，需要申请免试。请问申请公民时必须同时递交648表格吗？

一般的家庭医生可以填这个表，还是必须专科医生才行？谢谢
-Probably your father has to conduct the physical exam by the licensed physicians (专科医生) and then, the doctor will complete this form for him.
