按照693表格的要求，第6页，体检表应该是装在信封里，
On the back, write your initials across the seal where the flap meets the envelope.
Seal the entire flap with clear tape. Make sure the tape covers your initials as well as the flap.
我找的体检医生，在封口处盖了他们诊所的章，（有诊所地址电话）没有医生的INITIAL，然后也没有在这上面再帖上一层胶带。我当时问过他们，他们说不用，说他们一直这么干。回来想想总觉得不踏实，我需要再找他们去更改一下吗？
我去的那天到还有一对夫妻也是去体检的，不知道是不是真地就如他所说，体检人很多，一直这么干也没问题。
