老师，Part 11 上说：if you answer "Yes" to any of these questions, include a written explanation on an additional sheet(s)...。Question 7 A问的是Have you ever not。

--You are right. I missed the "ever not". I have corrected my answer.

如果答YES，不是还要寄报税的材料？好像没听说有人寄这些。

请8老师确认一下。
