strictly, by law, she is no longer in H4 and has to regain H4 by re-entering with H4 visa once I-485 denied.
In practice, there may be ways to stay without leaving, but no 100% gurantee (ways could get denied or officers doing a detail check would find out). The safest way (the only risk is not getting H4 visa stamp, which is low risk) is to re-enter.
