请问各位老师，I-90 问题，

绿卡寄到邮箱后被偷走。给移民局打电话，被告诉需要递交I-90表，而且在Reason for Application 选  “2.a. My previous card has been lost, stolen, or destroyed.“

问题是：

Part 3. Processing Information
Item Number 2. Location where your immigrant visa was issued or USCIS office where you were granted adjustment of status. 

网上说这个location 在绿卡上可以找到，可是我们见到绿卡前已经被偷走了。请问怎么回答这个问题？

I-797， Notice of Action -- 绿卡被批准的信是从“USCIS， DHS，  Lee‘s summit ，MO 64064 ”寄来的，能填这个吗？

“Item Number 1. Location where you applied for an immigrant visa or adjustment of status. “的地址是Chicago.
