请问您几个问题

1.填写的第一项1. Name of Alien中有一个Maiden name和Middle name是不填写还是写NA，Maiden name好像是女士才填写的。

-Leave it blank if it does not apply to you.

 

2.填写的第7项 Address in the United States Where you will Reside. 我现在在国内，不在美国，我也不知道将来会去哪里，这一条怎么写？

-If you don't have it, leave it blank.

3. 填写的第8项 Name of Address of Prospective Employer of alien has job offer in U.S. 这一条是不是也可以不填写

-If you don't have one, leave it blank.

4.最后麻烦问您，这个表格是打印出来填写

I found an online fillable one, and you may check whether it is the same vresion as the one above at DOL website. If yes, you may just fill it online and print it:
