通知是：Please submit Form ETA-750B, "Statement of Qualifications of Alien" or Form ETA-9089, "Application for permanent Employment Certification". Parts J, K and L.

-You need to provide the completed Form  ETA-750B by yourself for your NIW, not Form ETA-9089.  ETA-750B is for sefl-petition (NIW) and not need DOL approval.  But Form ETA-9089 (PERM) is filed by the employer for regaular EB-2 and EB-3, and has to be approved by DOL.

So, just complete the Form ETA-750B and provide it for the RFE.
