I-485：

Part 1，

I-94 #， 9 digits, space, 2 digits？

-It does not matter.

Part 2，

I am the derivative of my mom and she just got her green card. For part 2, I should not check a. but check b. (My spouse or parent applied for adjustment of status or was granted lawful permanent residence in an immigrant visa category that allows derivative status for spouses and children.)?

-Right.

Part3,

Were you inspected by a U.S. immigration officer?

answer YES?

-Correct, must say "Yes" unless you entered US illegally.

G-325A:

Applicant's last address outside the U.S. of more than 1 year: 详细的地址，including street name, number, apt #?

-Report it there as required.

Applicant's employment last five years: should I include graduate assistant job, other on-campus jobs under my F-1 student status?

-No.

Other supporting documents:

should I provide a copy of I-130 receipt notice?

-Not necessary, but provide the copy of your mother's green card (both sides), and your birth certificate.

I am currently approved of OPT, should I provide a copy of I-797C receipt notice and my EAD?

-Yes, and also provide the copy of your I-20 which covers your OPT period and the copy of your F-1 I-94.
