谢谢8老师昨天的回答。我还有几个问题请教8老师：

1. 填表的时候，我5年内工作单位只有一个，那表上的工作单位2，3是不填，还是每栏都填n/a？
-Leave the others blank because you have indicated your work from what date to what date in the employer which covers the period of 5 years.

2. 同样，part 4. information about your residence，只有一个，其他的2，3，4是不填，还是没栏填n/a？mailing address 和居住地一样，是不填还是填n/a？如果填n/a，c/o那栏不让填“/“？
-Same as above.

3. 表格必须用大写字母填写吗？
-Not need unless you fill it in handwriting.

4. 交材料的时候需要准备cover letter 吗？
-Not required.
