-Yes, only put your mother's name in "a" of the table.

2.十几年前办的出生公证和婚姻公证，现在还可以接着用吗？
-Yes.

3.I864和I864a及financial documents and supporting documents，可以在交完AOS fee之后马上提交吗？还是说必须要等付完IV fee并提交visa application form之后再交？
-NVC should send the instructions and you just follow it.
