目前F4亲属移民 primary beneficiary 已进入美国，我作为derivative beneficiary申请了F1-OPT，但是EAD card还没下来。现在我已经超过21周岁，但是child CSPA age是19岁。今年三月底收到的NVC寄来的instruction里也有我的名字。请问，我还需要重新notice NVC我还在这个case里吗？

从USCIS提供的I-485 instruction看来，我似乎需要准备以下材料（请老师指正）：
1. criminal history OR police clearance是一样的吗？本人没有任何犯罪记录。请问去哪个部门开这个证明？需要中国的证明吗？
-Not need this document for I-485 filing.
2. birth certificate
3. copy of passport page with nonimmigrant visa
4. photos (name with A number)我没有A#,但是我妈妈有，是否写上她的A#？
-Not need A# because you don't have it.
5. medical examination report.请问我需要在美国重新体检？
-Yes, need to provide the I-693 which is completed by a Dr. Find a Dr. at this website:

还是可以用以前在中国的体检报告？
-No. But show the examination report you got in China to the Dr. in US, then the Dr. may use some vaccine results for the new report, thus you don't need to inject this kind of vaccine.
我有两份med report，是几年前以nonimmigrant身份出国时，在中国的体检报告（1.international certificate of vaccination or prophylaxis；2. certificate of health examination）
6.Form G-325A Biographic info
7.Relative's addidavit of support,
-It means I-864 with the support documents (same as the one for you parent).
8.Evidence of eligibility (primary beneficiary's green card/immigrant visa?)
-Yes (both sides of the green card).
9. The cop[y of your I-20 with F-1 I-94.
请问这些材料都是和I-485一起递交吗？
