请问8老师，I-130的问题

在填 I-130 for my father, I encountered a question.

The background is: 
(1) Now, I am filing I-130 petition for my father
(2) I had filed an I-130 petition for my mother several years ago.
(3) When I got my green card, my wife also got her green card as my dependent.  
(We were married before I started applying for green card)

The question is related to I-130 D-2:
Have you ever before filed a petition for this or any other alien?  If "Yes", give name, place and date of filing and result

My Questions are:

(1) I have no question that I should list my I-130 petition for my mother in D-2.  
But has question on place and date.

give name ( my mother's name )
place ( what is the place?  the place where I reside when filing my mother's I-130? )
date of filing ( can I use the Priority Date on my mother's I-130 receipt ?)

(2) Just to be sure, that my wife go her green card as my dependent when I got mine should NOT count as a separation peition, right?  Therefore, I should not list my wife in the answer to D-2.
