Having been having issue to get DS-260 form on www.nvc.state.gov

Do we have to fill this form online? If not, is it possible for anyone to send me a copy of blank form.

The issue on NVC site has been going on for over a month. I have sent an email to NVC center asking for an electronic copy of this form, still waiting for response.
