Read Form I-485, Part 1, "Date of Last Arrival", which means your most recent entry to the United States. So, you only need to provide the evidence to prove your status since that date such as the I-94, visa page and entry stamp page of the passport, and any I-94 you got from the status extension or status change and the related status approval notice within US since that date.
You don't need to provide the status proof before your last entry. This is common sense: after you left US, your status was over (equal to that you have never come to US). Then, when you re-entered US again with a visa, you got a new status/I-94 (equal to that you came to US first time).
