我父亲上次来美延长2个月签证。I-797A 表写了valid date from 03/05/2013 to 05/01/2013. 然而护照本上没有这个信息。护照本上的签证issue date 是06/12/2012。

在DS-160 表上，我该如何填last visa issue date?
-Visa issue date means the date your father got the visa in the US Consulate last time (irrelevant to the extension of B-2 status).

因为是中信代签，可能不收I-797A 表，所以担心签证官认为我爸逾期未准时回中国。
-Your father needs to provide the copy of the I-797A for the new visa application, and if he can not use 中信代签, then he has to go for the visa interview personally.
