请问8老师，父母境内申请绿卡中，现在要求再次出示父亲和我本人的出生证明

信里主要内容是要这个：
Chinese Notarial Certificates are considered to be secondary evidence. Therefore, at least one of the following official documents must be submitted in support of the notarial certificate of birth:
- Birth Certificate
- Hospital Certificate
- Family Household Registry
- Family Household Register Form
- Booklet issued by the Province's Health Department regarding the birth
我父亲的证明应该就是户口本吧？
但是我本人的户口本那一页还是出国前的纪录（也就是说是错误的信息），这样也能公正户口本吗？
另外我本人的医院出生证明没有我的名字，只有妈妈的信息，和记录何时生产的一名婴儿。 是否我重新让医院开的出生证明里需要有父亲的名字？
现在很着急，因为要87天把证明寄给移民局。多谢帮忙！
