版上的老师们帮我看看几个给父母办绿卡填表的问题

我在一个月前已经给父亲递交了绿卡申请。现在给妈妈申请。希望版上的老师们帮我看看几个地方该怎么填？

Part D, #2, Have you ever before filed a petition for this or any other alien? If yes, give name, place and date of filing and result
 

place和date怎么填？date是应该填收到的I-130的receipt notice上写的时间(notice date or received date)吗？result is pending?

Part 3, #2

给父亲申请的时候，这里我是空着的。现在给母亲申请，需要check吗？因为父亲只是在1个月前刚交的申请。

已婚，加两个小孩，

给父亲的申请是填的时候，#1&#3是1，#4是2， #8是5，其他空着。现在给母亲应该还是这样填，对吗？因为父亲的申请只是交了一个月而已，还没有批下来。
