Form I-130 部分
- Completed Form I-130 and Form G1145.
- Check of US$420 payable to the “U.S. Department of Homeland Security”.
- My Notarized Birth Certificate .
- My parents’ Notarized Marriage Certificate. (For father only)
- Copy of my Naturalization/Citizen Certificate.
- Copy of my U.S. Passport.
Form I-485 部分
- Check of US$1070 ($985 for Form I-485, $85 for Biometrics) payable to the “U.S. Department of Homeland Security”.
- Notarized Birth Certificate.
- Copy of new and old Passport (including all non blank pages). -Information pages including the visa page are enough. If the visa for the last entry is on the new passport, then do not need the copy of the old passport. If the latest visa is on the old passport, just need the copy of the visa page of the old passport, not need the other pages.
- Copy of I-94 card (both sides).
- 2 Photographs.
- Completed Form I-693 in sealed envelope.
- Completed Form G-325A.
- Completed Form I-864 with supported documents provided by sponsor:
- Employee status and income verification letter.
- 2012 Tax return form 1040.
- W-2 forms of 2012, 2011, and 2010.
- Paychecks in the recent 6 months.

另外还有几个小问题，我会把 I-130 部分的申请和 I-485 部分的申请放在一个信封里（父亲一个信封，母亲一个信封），然后两个信封放一个package里寄出。
