回复：急，紧张。。。公司收到DOL perm certified的regular mail，但是没有找到9089表格？？？

今天早上，公司收到了DOL perm certified的regular mail，信上说，FORM 9089 is enclosed。 但是我们翻遍了整个信封，什么都没有，就两张纸；一个是说地址，如果寄错了，要return；另一个就是说dol certified perm，然后9089 enclosed。

但是没有任何form 9089在信封里。我打电话给律师，律师。。。在度假。。。下星期才回。

有经验的，大家说说看，是怎么回事啊？
-It seems DOL forgot to include the certified Form 9089 in the mail. Ask your HR/lawyer to contact DOL.
