My wife didn't take MMR vaccination when she took medical exam for the I-485 submission in Jan. 2012. We just received RFE asking her to "take Form I-693 with this notice to the civil surgeon so that the appropriate vaccination be administered". Would you please with the two questions below?

Should my wife take MMR only since that's the only one left out last year,
-Yes.

or she has to take all vaccination again?

If she takes MMR only, how will the surgeon comment on the completeness of the exam?
-Show the RFE to the Dr. and he/she will take care of it. You must find a Dr. who is assigned by USCIS  to fill the MMR result in the I-693 as you did last time.
