父母移民 485 part5 applicant statement 需要说我是翻译 我代填表吗？

Should I choose for my mom:

Each and every question and instruction...has been read to me in the Chinese language,...and put my name done in the Interpreter's Statement and Signature and also sign for Signature of Person Preparing Form
