DS-3032这个表格，是我填写。我是我妈填写[暂时没听说我妈那边有收到这个表格]，表格是不是要受益人[我妈妈]签字。如果需要她签字。

是不是要把这份表格寄回中国。还是说在网上下载一份在我妈直接打印出来填一下寄出去就行。

-Yes and then, print it out and sign it. Then, scan it and email back to NVC or email it to you, and then, you email it to NVC if you choose electronic processing. Otherwise, mail it to NVC.
