Form I-130 Petition for an Alien Relative

Check or money order, payment for filing fee

Evidence of petitioner’s U.S. citizenship, such as: birth certificate, U.S. passport, naturalization certificate

Copies of petitioner’s and beneficiary’s birth certificates, showing the name(s) of at least one common parent

Evidence of legal name change of either petitioner or beneficiary, if this has occurred
