J-1豁免表DS-3035在线填写的问题，不胜感激

"Is there any period of time in the US that is not covered by DS-2019 or IAP-66 form"

After my J-1 program finished, I re-entered US with F-2, then H1-B. DOES this question ask me to list the days after J-1 period?

"A statement demonstrating why the exchange visitor is eligible to receive a waiver of the two-year home country requirement of the exchange visitor program. The length of the statement may vary"

Normally, what should be included in this statement?

Since J-1 already fulfiled the 2 year HRR (want to apply J-1 waiver for J-2) , what should I say in the statement?
