Help on I-765 application

Hi. Please help.

I have submitted I-485. I am renew my EAD. I used my AP last August. On my AP and I-94, the status is AOS. My question is:

should I fill in AOS at both #14 (Manner of Last Entry) and #15(Current Immigration Status) in I-765 application?

I am at Texas, should the address of the application is:

PO Box 660867
Dallas, TX 75266
