-Only need the information pages including the visa and entry record pages.

Picture和biographic页要彩色的3份？

-Not need color copy. One copy for I-485.

US Visa和进出US日期页要黑白一份？

几年前老旧过期的护照相关页也要吗

-Not need unless the visa for your last entry was in that passport (only need to provide the visa page copy in this case).
