These three parts are confusing to me. My questions is: If one violates D and E, and is a spouse of a US citizen, can s/he apply for the status of adjustment? - Yes  (please look up "unless" in an English dictionary)

Is part D or E is more serious than "you failed to maintain your nonimmigration status"? What exactly "You failed to maintain your nonimmigration status" means?
