I recieved the letter regarding the information below:

1) Birth certificate: As we all know, we don't have birther certificate in China, I might still have the notary that including my name, my parent's name,

-Birth certificate should include your name, your parents' names, your birthdate, and your birth place.

 but it was done in 1999, can I use it?

-Yes.

 I tried to find more info on state.gov, but no luck so far.

2) Med exam and Vaccination Record: Same here, we don't keep vaccination record when I was in China, how do I deal with this?

-The physical exam report (I-693) has to be done within US. Have you provide the I-693 with your I-485 filing yet? If No, you need to do it and provide it to USCIS.
