Sure, it's simple.
i took the printout of the FOIA G639 form and filled it with all my possible details.
I attached a cover letter stating that my emp didn't provide me with a copy of perm and I140 and send it with USPS priority mail.
after a week or so i got the confirmation that my request has been received and to proceed further they want my parent's info ( on my passport)and my last port of entry date.
I send the same info again in the form of notarized xerox copy of my passport along with letter which they sent to me which have the control # mentioned.

Finally just after 30-31 days, i got the CD from USCIS which have the PDF versions of my labor & approved 140.
The PD is been mentioned on the approved 140.

Friends dont loose hope even if you dont have receipt # or any other info.
just send everything using USPS priority mail to avoid any delay as we need to submit the demanded doc within 30 days of getting the letter.
