那我这个算是新申请呢,还是换卡申请?该填哪种表格呢?

-Just bring your old SSN card, your green card and your ID to the local SSN office and tell them you want to repalce the SSN card (without the above words on the card), then they will give you a form (Form SS-5) to complete and then they will take care of it.
