急问：typo I-693 medical report

体检的医生在填表时，抄错了我 TD 疫苗的 received date. 我应该回去让医生correct the typo 吗？ 还是没关系？
另外，In part 1, 医生预先填了我的signature date as my first examination date, rather than the final follow-up examination date. 这有问题吗？
