母亲病重，我于11月申请AP想回去探望，一个多月后还没拿到，于是我预约了INFORPASS,要求办理加急AP,可是移民局的人告诉我这种情况需要我母亲所住医院的证明才行，当时只记得他说要有医院的地址和电话，可是国内医院的大夫说没开过这种证明，问有没有具体的格式和内容。
我想请教是否有人在国内开过这种证明？格式和内容？
