how do i put chinese names in the gc forms?

say a chinese name WANG Xiao Ya, do I put Xiao as first name and Ya as middle name, or do i put both Xiao Ya as first name? Does Xiao Ya have a space in between? Or should I just put XiaoYa together without a space as a first name? thanks for your input!!
