It's the first time to use AP come back to US, current status is AOS, using EAD work. Please advise what kind of documents I need to take with me

-I-485 receipt and your passport.

and what status I need to fill in the form of I-94.

-Nothing. The CBP officer at the port of entry will indicate your status in the I-94, not by you.
