why my I-140 approval noticed

is sent to NVC? The immigration petition is approved by the INS, but my elegibility to adjustment of status in USA is denied and the approval notice is sent to NVC. My H1 and H1 extension are both approved without gap in time. I just did not sent the H1 extension approval letter to INS before 140 was processed. Do I have to send the H1 extension approval letter to INS in order for them to know my valid visa status?
Does anybody has any idea or similar experience?
