The Dr. can not send it separately unless you get the RFE later. If the Dr. send it until you get the RFE, this will delay your case process. Normally, the Dr. just needs to seal the report and sign it crossing the sealed part on the envelop, and gives the sealed envelop/repaort to you, and you send all the documents in one packet to USCIS. If the Dr. send the report separately before you get the RFE, USCIS will throw the report away into the trash can. So, discuss this with the Dr. I really doubt the Dr. has expereince in this kind of thing.
