亲属移民的工卡申请必须寄往伊利诺伊州芝加哥的申请邮递中心, 而职业移民按照申请人居住地址不同，则寄往其归属的申请邮递中心，如居住在加州的职业移民申请人需要递交工卡延期申请至亚利桑那Phoenix 的申请邮递中心；如果是居住在纽约的职业移民申请人，其工卡延期申请需递交至德州Dallas的申请邮递中心。

File at the USCIS Chicago Lockbox facility if your response to question 16 is:

(c)(9) and you filed your Form I-485 with the USCIS Chicago Lockbox facility, and your recipt number begins with "MSC". You must include a copy of the I-797C, Notice of Action, which show your Form I-485 was accepted.
-This means if your I-485 was filed to the Chicago Lockbox facility (neither TSC nor NSC, e.g.family based I-485), you should send your EAD application to Chicago Lockbox facility.

File at the UScIS Phoenix or Dallas Lockbox facilities based on where you live, for all other Form I-765s.
-If your I-485 was filed based on your EB I-140, you should send your EAD application to on where you live. In your case: the USCIS Phoenix Lockbox facilities (see page 10)
