请问大伙儿 那个 I-751的 affidavits

我在准备I-751的材料，看到填表说明书上说要有affidavits sworn to or affirmed by at least two people ... 心里挺疑问的，想问问大家：

1、这个 affidavits 是必须要提供的吗？

2、如果这个 affidavits 是必须的，大家有谁有这个文件的“模板”或者 “格式” 吗？文件要有落款签名吗？ 这个文件要拿去公证吗？
