Urgent help needed !!!

If I marry a girl who is on B1 visa, does she have to go back to China to get H4 (I'm H1 holder)? Is it possible to get H4 without leaving the States? What kind of document should we get prepared for getting H4? Your help will be highly appreciated. Thanks a lot.
